Important: Please read this waiver carefully
before booking. By agreeing, you acknowledge the inherent risks of
athletic training.
1. Assumption of Risk
I understand that participating in goalkeeper training activities
involves inherent risks, including but not limited to physical
injury, sprains, strains, fractures, concussions, broken bones,
torn ligaments, muscle injuries, and other injuries that may
result from training exercises, drills, diving, catching, jumping,
and other physical activities.
I voluntarily assume ALL risks associated with my
participation, whether known or unknown, and regardless of the
cause.
2. Release of Liability & Waiver of Claims
In consideration of being permitted to participate in goalkeeper
training sessions offered by Diego's Goalkeeper Academy,
I hereby release, waive, discharge, and covenant not to
sue
Diego's Goalkeeper Academy, Coach Diego, its owners, coaches,
employees, agents, representatives, and affiliates from
any and all liability, claims, demands, actions, or causes of
action whatsoever
arising out of or related to any loss, damage, injury, or death
that may be sustained while participating in training sessions,
traveling to/from sessions, or while on the premises where
training occurs.
3. Indemnification
I agree to indemnify and hold harmless Diego's
Goalkeeper Academy and its representatives from any claims,
damages, injuries, or expenses (including attorney fees) arising
from my participation in training activities or any breach of this
agreement.
4. No Liability for Injuries
I understand and agree that Diego's Goalkeeper Academy, Coach
Diego, and all associated parties are NOT responsible or liable
for any injuries, accidents, illness, or death
that may occur during, before, or after training sessions,
regardless of whether such injuries result from negligence,
equipment failure, facility conditions, the actions of other
participants, or any other cause. I accept full responsibility for
my own health and safety.
5. Medical Acknowledgment
I certify that I am physically fit and have no medical condition
that would prevent my full participation in goalkeeper training
activities. I agree to inform the coach of any physical
limitations, medical conditions, or injuries that may affect my
training. I understand that training will proceed at my own risk
regardless of any conditions disclosed.
6. Emergency Medical Authorization
In the event of an emergency, I authorize Diego's Goalkeeper
Academy to seek emergency medical treatment on my behalf. I
understand that I am responsible for all medical costs incurred.
7. Photo & Video Release
I grant Diego's Goalkeeper Academy and Coach Diego
full permission to photograph and record video of
me (or my child) during training sessions. I understand and agree
that these photos and videos
will be used on social media platforms (Instagram, Facebook,
TikTok, YouTube, etc.), this website, and for any other
promotional or marketing purposes
without compensation or further approval. I waive any right to
inspect or approve the finished content and release all claims
related to the use of my likeness.
8. Cancellation Policy
I understand that cancellations must be made at least 24 hours in
advance to receive a full refund or reschedule. Cancellations made
less than 24 hours before the scheduled session may result in
forfeiture of the session fee.
9. Code of Conduct
I agree to conduct myself in a respectful and sportsmanlike manner
during all training sessions. I understand that inappropriate
behavior may result in dismissal from the session without refund.
10. Privacy Policy
I understand that my personal information (name, email, phone
number) will be collected for the purpose of scheduling and
communication regarding training sessions. Diego's Goalkeeper
Academy will not share my personal information with third parties
without my consent, except as required by law.
11. Agreement & Acknowledgment
By checking the box and proceeding with the booking,
I acknowledge that I have read, understood, and voluntarily
agree to all terms outlined above.
I understand that this is a legally binding waiver and release of
liability. If booking for a minor, I confirm that I am the parent
or legal guardian and have the authority to agree to these terms
on their behalf.
